Open Water Rx was built to change that. Our 100% telehealth model means you never have to take time off work, arrange childcare, or drive across town to see a provider. Your entire evaluation, prescription, and ongoing care happens from your phone or computer, on your schedule. No insurance required. Discreet. If medication is prescribed, it ships directly to your door. Shipping timelines vary by program, pharmacy, and location.
We serve adults who have tried diet and exercise and found that those tools alone were not enough. We serve people who have been told by their primary care provider that they should lose weight but were never given a clear, medically supported path to do so. We serve people who have heard about GLP-1 medications like semaglutide and tirzepatide and want to understand whether they are a clinically appropriate option for their specific situation.
We do not serve everyone. Every patient is evaluated by a licensed medical provider before any prescription is issued. If a provider determines that GLP-1 therapy is not appropriate for you, we will tell you clearly and, where possible, point you toward other resources.
GLP-1 therapy is generally considered for adults with a BMI of 30 or higher, or a BMI of 27 or higher with at least one weight-related health condition such as type 2 diabetes, hypertension, or high cholesterol. Your prescribing provider will review your full health history to determine whether GLP-1 therapy is appropriate for you.
Open Water Rx serves patients across all 50 states. Some states may require a brief live video visit with your clinician before a prescription can be issued. We will let you know if that applies to you.
Most patients complete their intake questionnaire in under ten minutes. A licensed medical provider reviews your evaluation asynchronously after submission. If approved, your prescription is sent to a licensed compounding pharmacy and your medication is shipped directly to your home. Timing varies by state, program structure, and availability.
The Science Behind Our Approach
For decades, the medical community treated obesity primarily as a behavioral problem. The prescription was simple: eat less, move more. For many people, that advice was not only unhelpful but actively harmful, reinforcing the false idea that weight was purely a matter of willpower. The science has since caught up with what many patients already knew: obesity is a complex, chronic, multifactorial disease with significant genetic, hormonal, and neurological components.
GLP-1 receptor agonists represent the most significant advance in obesity medicine in a generation. Originally developed to treat type 2 diabetes, drugs in this class were found to produce substantial, sustained weight loss in clinical trials at a scale previously only achievable through bariatric surgery. The STEP 1 trial, published in the New England Journal of Medicine in 2021, found that adults treated with semaglutide 2.4 mg lost an average of 14.9% of their body weight over 68 weeks, compared to 2.4% in the placebo group. The SURMOUNT-1 trial for tirzepatide, published in 2022, showed even more striking results, with participants losing up to 22.5% of their body weight.
These medications work by mimicking naturally occurring hormones that regulate appetite, gastric emptying, and blood sugar. They reduce hunger, increase feelings of fullness, and help the body process glucose more efficiently. Critically, they address the underlying hormonal dysregulation that makes sustained weight loss so difficult for many people, not just the symptoms.
At Open Water Rx, we believe that access to these medications should not be limited to people who can afford concierge medicine or who happen to live near a specialist. Our platform is designed to make clinician-guided GLP-1 therapy accessible to eligible adults who meet the clinical criteria, at a transparent monthly cost, with no insurance required and no waiting room.
